At Pokeno Family Health, we are proud to serve individuals in the Pokeno region. We provide comprehensive healthcare services to a diverse range of patients, including children, pregnant women, working families, and those with chronic conditions like diabetes and hypertension. We are committed to ensuring that everyone has access to quality healthcare, especially those who find it challenging to obtain medical services.

As part of the Alliance Health + Primary Health Organisation (PHO), we benefit from government funding on an annual basis, which helps subsidise your healthcare costs. This funding model allows us to offer more affordable care to our patients.

Enrolling with one of our GPs is free. Once enrolled, your consultations will be subsidised, resulting in lower fees for you. Please keep in mind that the approval process for subsidised funding and enrolment may take a few months.

You are eligible for subsidised funding if you are a New Zealand citizen, a permanent resident, or if you hold a work visa for at least two continuous years.

HOW TO ENROL

We have simplified the enrolment process to make it hassle-free for you. We require patients to complete the registration/patient form before your first appointment can be scheduled.
In order to complete your enrolment and obtain government funding for subsidised healthcare we are required to sight one or more (see below) of the following original documents
To verify your address, we also require 2 separate letters/documents, with your name and address on them. (E.g. bank statement header, phone, or power bill).
NZ Citizens
• Passport, NZ Birth Certificate or Certificate of Citizenship.
• If only a Birth Certificate or Certificate of Citizenship is available, we will require another form of photo 1.0 (e.g. Driver’s License), to confirm your identity.
NON-NZ Citizens
• Passport with Permanent residency Visa or work Visa stamp(s) covering at least 2 consecutive years.
Note:
If you have less than 2 years of Visas you will NOT qualify for Government subsidised health care. We will therefore have to charge you as a visitor to New Zealand.
Please ensure that all of your details are correct, including medications. This would assist us in completing your enrolment in a timely manner.
